ABSTRACT:
Introduction: India faces significant challenges in providing access to basic healthcare and necessary medications to its large population, despite the remarkable expansion of the country's pharmaceutical sector. About 20 percent of drugs that are procured fail in the quality control tests, impacting public confidence. With a focus on the need for multifaceted assessments to improve drug availability, quality, and public confidence, this study aims to evaluate the efficiency, effectiveness, and quality of public drug procurement systems across seven states in India by developing a specific set of performance indicators. Methodology: Using 22 predetermined parameters that focus on efficiency, cost-effectiveness, and quality, a quantitative study was undertaken to evaluate the performance of public drug procurement systems in seven sample states during the year 2021-2022. Data was collected using a data collection tool, telephone interviews, and references to the respective medical corporation's website. The whole data collection process and the scoring framework were validated by performing a pilot study on two of the study sample states to ensure robustness across diverse regional states. Findings: The study revealed considerable differences in efficiency, cost-effectiveness, and drug quality-related performance parameters. Among the sample states, Tamil Nadu and Kerala exhibited superior drug procurement performance. Conversely, states like Odisha and Chhattisgarh faced challenges in budget allocation and compliance, underscoring the need for policy reforms. This study also highlights the need to consider geographical and financial factors for a comprehensive evaluation and enhancement of public drug procurement efficiency throughout India. Conclusion: This study examines the strengths and weaknesses of public drug procurement processes in seven states. The study also underscores the necessity for policy reforms and collaborative initiatives among various stakeholders to improve procurement processes. By implementing comprehensive evaluation methodologies and adhering to a benchmark model, policymakers can reduce substandard medications and improve public health outcomes.
