ABSTRACT:
Background and Purpose: Though all healthcare professionals are aware of the term “Medication reconciliation” and its importance in maintaining medication safety, individualized training in medication reconciliation and incorporating this process in routine clinical practice needs to be assessed among healthcare professionals at regular intervals for optimal patient care. Methodology: This study utilized a pre-test questionnaire on medication reconciliation among healthcare professionals followed by an educational intervention session on medication reconciliation. The improvement in Knowledge, Attitude and Practice after the training session was assessed using a post-test questionnaire. Results: Major proportions of the participants were aware of the term “Medication Reconciliation” and the stakeholders involved in the above process. But they were not fully aware of the key roles medication reconciliation plays in improving patient treatment outcomes and minimizing medication errors and adverse effects at transitions of care. However, after the educational intervention, there was a statistically significant increase (p <0.05) using paired t test and Wilcoxon signed rank test in the participants’ Knowledge, Attitude and Practice towards Medication Reconciliation. Conclusion: The overall knowledge on medication reconciliation and precise application of its practices were greatly improved following the educational intervention session compared to the baseline. Medication reconciliation must be practiced at every point of care transition to promote patient safety in clinical practice.
